Who benefits from Targeted Case Management?
Targeted Case Management is designed for Medicaid members who have been diagnosed with a qualified mental health diagnosis, which can include social and emotional disorders.
The purpose of Case Management for mental health is to make it easier for a person or family to determine what services are available to them and how to take advantage of them. While there are good programs in place to help, it can be hard to figure them out on your own.
What does a case manager help with?
At Best Life Louisville, your Targeted Case Manager will help you find and use the resources available to you. This includes assisting you with things like:
- Getting a psychiatric evaluation and review
- Completing a comprehensive assessment
- Medication management
- Creating a crisis intervention plan
- Reinforcing and encouraging the use of skills learned through therapy
- Working within school systems to advocate for your child’s needs
- Finding and connecting with Community Support Associates, Therapeutic Groups, and Occupational Therapy
- Accessing community services including health and dental care, housing assistance, and Supplemental Nutrition Assistance Program (SNAP) benefits.
The role of your Best Life’s case manager is to help you make sure your or your loved one’s needs are met. You will meet with your case manager regularly so they can provide ongoing assistance and reassess to make sure adjustments are being made as needed.
